Ectopic Pregnancy 101

An ectopic pregnancy refers to the implantation of a fertilized egg outside the uterus.  The word ectopic means ‘out of place.  In 95% of cases of ectopic pregnancies, the egg implants in the fallopian tubes, which is why they are also commonly referred to as tubal pregnancies.  However, a fertilized egg can also implant in the cervix, in the ovary or even in the abdomen.  These cases are referred to as cervical, ovarian or abdominal ectopic pregnancies.

An ectopic pregnancy is a critical condition for the mother.  As a fetus develops, it will outgrow the space where it is implanted and cause the organ to burst.  When this happens, severe hemorrhaging will occur, threatening the mother’s life.  In the meantime, the fetus is deprived of oxygen and blood supply and dies.

Any woman can have an ectopic pregnancy, since it mostly depends on the failure of the fertilized egg to settle inside the uterus.  However, the risk for ectopic pregnancy is higher among women who are over 35 years old, and with previous history of pelvic inflammatory disease (PID), fallopian tube surgery, problems with infertility, use of ovulation-stimulating drugs, or a previous case of ectopic pregnancy.  Some birth control methods such as intrauterine devices (IUDs) can interfere with the normal travel of a fertilized egg; while the odds of fertilization is rare with an IUD, a fertilization that does occur is likely to implant outside the uterus.

At the start, an ectopic pregnancy often does not produce any symptoms different from a normal pregnancy.  Breast tenderness, morning sickness, frequent urination and missed periods are all symptoms experienced by pregnant women.  Often, the first warning sign of an ectopic pregnancy is a sharp stabbing pain in the pelvis or abdomen accompanied by vaginal bleeding.  This is actually a signal that the ectopic pregnancy has already caused an internal rupture.  This diagnosis is strengthened if any of the following symptoms manifest: lower back pain, low blood  pressure, dizziness or fainting possibly due to blood loss, and vaginal spotting.

The diagnosis of an ectopic pregnancy can be confirmed by an ultrasound examination.  However, any pregnancy less than 5 weeks after the last menstrual period can be hard to visualize even with good ultrasonic equipment.  This is one reason why ectopic pregnancies are not easily detected in its early stages.

The Controversy of Compounded Bioidentical Hormones

Due to the recent press about bioidentical hormones, a lot of women have become curious about them, Oprah has been talking about it on her show and Suzanne Somers is a well known advocate, but still not is know about compounded bioidentical hormones. I’m sure most women who are of menopausal age are well aware of the risks associated with traditional hormone replacement therapy (HRT). One study started in 1991 by the Women’s Health Initiative on traditional HRT showed many increased risk factors associated with the estrogen only HRT as well as the HRT with both estrogen and progesterone which is called a combined HRT, both of these therapies sharply increased the risks of breast cancer, heart disease and strokes.

With traditional HRT, a specific dosage of hormones is given to each woman; because the medications (hormones) were commercially produced and regulated they could then be regulated by the FDA. Now FDA approval is no guarantee of effectiveness or long term safety as time has proven but at least this has given a platform of uniformity on which to conduct clinical trials. Herein lies the problem with compounded bioidentical hormones (BHRT), these BHRTs are made individually according to each woman’s needs.
